NOTICE: Only fill out this form if you have been directed by a representative of the Riverworks Financial Clinic. Your Riverworks Financial Clinic representative will inform you what documents you need to submit, below is a general list.

• Intake Form, W-2, Voided Check, Past Tax Return, Social Security Card, Picture of I.D. 

• Other documents you might need to submit: interest forms, profit and loss statements, donations, etc.

SECURITY: At the Riverworks Financial Clinic the security of your private documents is our number one priority. That's why we partnered with Jotform.com, a company that prioritizes security above all else. Our form is served across a  protected Secure Socket Layer connection that uses a SHA256 Certificate. This is the same level of protection used by online banking or e-commerce providers.

When users upload files using this form, their files are stored on secure servers and only accessible to the Riverworks Financial Clinic staff.

SAVE: We understand that it takes time to gather all the documents required to complete your taxes, use the "SAVE" button at the bottom of the form and pick up right where you left off. You will receive an email link allowing you to resume at your convenience.

SUBMIT: After you submit the required documents, you will receive an immediate confirmation email. It will take the Riverworks Financial Clinic 24 to process your taxes and follow-up.
